Jacob hat kleine dicke Hände, sein Gesicht ist eine verrückte Dauergrimasse und ständig lacht er völlig grundlos. Paul findet seinen jüngeren Bruder "zum Kotzen". Jacob hat Down-Syndrom. Der Umgang mit ihm erfordert viel Geduld und ablehnende Gefühle sind verboten. Mit einem immer währenden "Jacob-Lächeln" kümmern sich die Eltern um ihr behindertes Kind. Doch wo nicht abgelehnt werden darf, kann auch keine Zuneigung entstehen. In kurzen, immer wieder rückblickend Episoden, erfährt man im Wesentlichen aus Pauls Perspektive dessen Versuch, sich das eigene Leben "ohne Jacob" zu denken. Es sind Schlüsselszenen, die die Beziehung der Geschwister beschreiben: die Ankunft des Babys, Jacobs erste Geburtstagsparty, den Umzug von New York aufs Land. Die Stärke des Buches ist die komplexe Darstellung dieser Gefühlsverbiegungen. Auf ein hundertprozentiges Happy-End muss noch verzichtet werden - dennoch kommt es zu einer ersten Annäherung zwischen den beiden Brüdern.
